Diver Medical
For the health and safety of you, our staff and other divers and for insurance purposes; before going on any SCUBA Diving Courses or Trips all divers must complete the medical questionnaire shown below.
If you answer ‘Yes’ to any of these questions then you must complete the second form shown below with more details.
DAN Medical
If you answered ‘Yes’ to any of the three questions on the previous form then please complete the following DAN Medical questionnaire as well.
Please note it is 2 pages of questions for you to complete. The 3rd page is the Medical Examiner’s Evaluation Form to be filled by a physician, if you need to have a check-up to be cleared fit for diving.
