For the health and safety of you, our staff and other divers and for insurance purposes; before going on any SCUBA Diving Courses or trips all divers must complete the medical questionnaire shown below.

DAN Medical Questionnaire

Carefully read all of the questions on the first page and answer as accurately as you can.

If you tick ‘No’ to all, only the first page needs to be signed.
If you tick ‘Yes’ to one or more health issues on the first page, you need to complete the second and third pages. The third page, the “Medical Examiner’s Evaluation Form” requires a visit to a doctor, a check-up to be cleared fit for diving and the doctor’s signature on the form.
